Client fees for social welfare and health care services 2023
Some of the Wellbeing Services County’s services are subject to fees. Client fees can be the same for all clients or dependent on the client’s ability to pay.
The national upper limit for health care client fees is EUR 692. The national upper limit is subject to an index adjustment every other year.
The maximum client fees for many services are defined by Finnish client fee legislation. The maximum fees are subject to index adjustments every other year. Regardless of the defined maximum fee, the fee for a service cannot exceed the cost of providing the service.
Services that are exempted from client fees are defined separately in the legislation. For services that are not exempted from client fees, wellbeing services counties can decide on the charging of fees and fee amounts within the boundary conditions set by the legislation.
Client fees for social welfare and health care services are regulated by the Act on Client Charges in Health and Social Services (734/1992) and the supplementary Government Decree (912/1992). Social welfare and health care services provided by wellbeing services counties can be subject to fees unless otherwise provided by legislation.
Family care
Short-term family care
- persons over the age of 16: EUR 26 per day
- persons under the age of 16: EUR 11.60 per day
Part-daily short-term family care for seniors
(up to 8 h of daytime care or 12 h night-time care): EUR 13 per session
No separate meal fee.
Part-daily family care organised by disability services is free of charge for the client.
Long-term family care
The maximum fee is 85% of net monthly income. The client retains 15% of their net monthly income or at least EUR 167 per month for personal expenses.
The fees for long-term family care are charged in the same way as the fees for long-term 24-hour service housing.
Fees for long-term family care services are not charged to front-line veterans (Act on Front-line Veteran Rehabilitation 1184/1988) or for family care provided on the basis of the Child Welfare Act (417/2007), the Act on Special Care for People with Intellectual Disabilities (519/1977) or the Act on Disability Services and Assistance (380/1987).
Client fee for short-term 24-hour service housing
The client fee for short-term 24-hour service housing covers care, meals, support services, housing costs and other maintenance. The care is provided outside the customer's home and as non-institutional care.
- persons over the age of 16: EUR 26.00 per day
- persons under the age of 16: EUR 11.60 per day
Long-term 24-hour service housing
Long-term 24-hour service housing is subject to a client fee based on the client’s income. The client fee is 85% of the client's net monthly income, after statutory deductions. The client retains 15% of their net income or at least EUR 167 per month for personal expenses.
If the client lived in a shared household married or in conditions comparable to marriage immediately prior to the start of the service and their monthly income was higher than that of their spouse or partner, the client fee is determined based on the combined monthly net income of the client and their spouse or partner. In this case, the fee is 42.5% of the total monthly net income, after statutory deductions.
Communal housing for seniors
Fees for communal housing for seniors, in which the client is provided with continuous and regular service at home, are determined in the same way as the fees for regular home care. In addition, fees for meals and support services and rent are charged separately.
Meal package: EUR 430 per month
Individual meals:
- Breakfast EUR 1.60
- Lunch EUR 6.20
- Daytime coffee EUR 1.20
- Dinner EUR 6.20
- Light supper EUR 1.60
Support service fees:
- cleaning EUR 30 per month
- clothing maintenance EUR 36 per month
Temporary accommodation
Temporary accommodation for clients of mental health and substance abuse services: free of charge
Temporary substance-free accommodation for adults: free of charge
Housing services as part of mental health and substance abuse services
Communal housing
Short-term communal housing service: EUR 22.50 per day
Fees for communal housing, in which the client is provided with continuous and regular service at home, are determined in the same way as the fees for regular home care. In addition, the client is charged a separate meal service fee. The meal service fee is EUR 261 per month. The fee covers daily meals.
Communal housing was referred to as long-term service housing before 2023.
The client fee for long-term 24-hour service housing is up to 85% of the client's net monthly income, after statutory deductions. The client retains 15% of their net income or at least EUR 167 per month for personal expenses.
Demanding 24-hour service housing
In demanding 24-hour service housing provided as part of mental health and substance abuse services, the minimum amount of money that the client retains for personal expenses is equal to the basic amount of social assistance for persons living alone (EUR 532.97 per month in 2022).

Temporary home care and home nursing
Temporary home care:
Visits shorter than one hour: EUR 8.00 per visit
Visits longer than one hour: EUR 12.20 per visit
The maximum fee is EUR 24.40 per day.
Temporary remote home care:
Contact-based session fee EUR 3.30.
The maximum fee is EUR 16.50 per day.
Fees for temporary home care or remote home care are not charged if the client’s income falls below the income limit defined for home care services.
Temporary home nursing:
House call by a doctor or dentist: EUR 19.20
House call by other professional or client’s visit to the home care office: EUR 12.20
The maximum fee is EUR 36.60 per day. Fees for temporary home nursing count towards the annual upper limit for health care fees. The fees are not charged if the client is a regular home care client (section 3 of the Decree on Client Charges in Healthcare and Social Welfare). Front-line veterans are exempt from home nursing fees.
Home care and home nursing assessment visit: free of charge
Preventive examination visit: free of charge
Regular home care
Regular home care is subject to a monthly fee based on the client’s income. The fee for regular home care is charged if the service has been used for at least two months or if it is estimated that the service will be used for at least two months and if the client uses the service at least once a week.
The fee for regular home care is determined based on the service hours specified in the service decision and the client’s ability to pay and family size. The monthly fee is determined in accordance with the table below. The fee is a percentage of the portion of the client’s gross income that exceeds the income limit. Income limits for exemption from home care client fees based on family size
- Family size 1 person – income limit EUR 598.00 per month
- Family size 2 persons – income limit EUR 1,103.00 per month
- Family size 3 persons – income limit EUR 1,731.00 per month
- Family size 4 persons – income limit EUR 2,140.00 per month
- Family size 5 persons – income limit EUR 2,591.00 per month
- Family size 6 persons – income limit EUR 2,976.00 per month
If the family size is larger than six persons, the income limit is increased by EUR 356 for each person.

Home rehabilitation assessment visit: free of charge
Support after hospital discharge: EUR 8.00 per session. The maximum fee is EUR 16.00 per day. Support after hospital discharge is provided for 1–5 days.
Rehabilitation and assessment period:
- EUR 46.20 per starting week
- EUR 24.00 per starting week (without treatment visits).
The duration of a rehabilitation and assessment period is approximately 4–6 weeks. Also includes remote visits.
Remote rehabilitation:
- 1–3 sessions: free of charge
- 4–6 sessions: EUR 11.60 per period
- 7 or more sessions: EUR 23.20 per period
Home rehabilitation therapy session at the client’s home: EUR 11.60 per visit
For home care and informal care support clients: free of charge
Temporary home service for families with children
The service is subject to a client fee based on the length of the visit.
- under one hour: EUR 7.00 per visit
- 1 hour – under 2 hours: EUR 10.00 per visit
- 2 hours – under 4 hours: EUR 17.00 per visit
- 4 hours – under 6 hours: EUR 21.00 per visit
- over 6 hours: EUR 27.00 per visit
Income limits for exemption from home care client fees based on family size
- Family size 1 person – income limit EUR 598.00 per month
- Family size 2 persons – income limit EUR 1,103.00 per month
- Family size 3 persons – income limit EUR 1,731.00 per month
- Family size 4 persons – income limit EUR 2,140.00 per month
- Family size 5 persons – income limit EUR 2,591.00 per month

The home hospital fee is EUR 22.80 a day.
The fee is not charged if the client is a regular home care client, receiving short or long-term institutional care or 24-hour housing services for seniors or being treated for diseases defined in the Communicable Diseases Act.
Assessment visit by the hospital discharge team’s nurse or physiotherapist: free of charge.
Home visit by the hospital discharge team’s nurse: EUR 12.20.
Home visit by the hospital discharge team’s physiotherapist: EUR 12.20
Services that support living at home are free of charge for front-line veterans.
Clothing maintenance service: EUR 8.50 per time.
Washing service: Washing service outside the home, without transport: EUR 8.50 per time.
Washing service outside the home, with transport: EUR 12.50 per time.
Cleaning service: EUR 30 per h or service voucher.
Shopping service with home delivery: EUR 9.50 per time.
Assisting services:
The service is subject to a one-off fee based on the duration of the service.
Service duration under 2 hours: EUR 7.40 per visit.
Service duration 2-4 hours: EUR 13.90 per visit.
Care alarm service: The fee is 3.5% of the client’s gross monthly income. The maximum fee is EUR 45 per month. The service is free of charge for the following client groups:
- clients whose gross income falls below the exemption limit
- front-line veterans
- clients who have been issued a housing service decision by disability services.
The care alarm service includes alarm equipment, 24-hour alarm monitoring and visits by a care worker. The locator bracelet service also includes the relaying of location data to a contact person and, if necessary, a door alarm. Additional equipment is subject to a fee in accordance with a separate price list.
Meal service:
- Home-delivered meal: EUR 10.50
- Vending machine meal: EUR 9.70
- Meal at the service centre: EUR 7.65
Physiotherapy and lymphatic therapy, occupational therapy for adults, speech therapy for adults, neuropsychologist consultation for adults, other therapies:
Personal session (including remote sessions): EUR 11.60
Group sessions and remote group sessions:
- 1–3 group sessions: total EUR 11.60
- 4–6 group sessions: total EUR 23.20
- 7 or more group sessions: total EUR 34.80.
Mobility support service in accordance with the Social Welfare Act
- Co-payment: EUR 3.10 per one-way journey.
- Co-payment for 7–17-year-olds: EUR 1.55 per one-way journey.
- The maximum reimbursement for taxi journeys is EUR 25 per journey.
Mobility support service in accordance with the Services and Assistance for the Disabled Act
Mobility support services in accordance with the Services and Assistance for the Disabled Act are subject to co-payments in accordance with HSL’s or Matkahuolto’s fare prices.
Medical certificate or statement issued by a doctor or dentist: EUR 51.50
Medical certificate for obtaining or maintaining the right to drive: EUR 61.80
No fee is charged for the following certificates, among others:
- certificates or statements related to treatment provided as part of public health work
- health certificates based on information collected in school or student health care
- certificate or statements necessary for access to treatment or rehabilitation.
Fee for services provided during an informal caregiver’s statutory leave: EUR 11.60 per day.
- The fee is levied on both adults and children.
- No separate fees for meals or other services included in the service package are charged in addition to the client fee.
- Services organised with a service voucher and clients whose informal care is part of a service housing at home service portfolio in accordance with section 8 of the Services and Assistance for the Disabled Act (1987/380) are exempt from the fee.
- Front-line veterans are exempt from the fee for services provided during an informal caregiver’s statutory leave.
Missing a scheduled appointment without an acceptable reason for not cancelling will result in the client being charged a missed appointment fee of EUR 51.50.
The missed appointment fee will be charged even if the appointment was part of a service that is free of charge. The missed appointment fee is charged to all clients over the age of 18.
These include the services of the Geriatric Assessment and Rehabilitation Outpatient Clinic, the Memory Outpatient Clinic, the Infusion Outpatient Clinic, the Follow-Up Outpatient Clinic and the Palliative Outpatient Clinic.
Assessment visit to a clinic: EUR 20.90. Includes a doctor’s appointment, a nurse’s appointment and a therapist’s appointment at the clinic.
Assessment appointment with a nurse or therapist: free of charge
Doctor’s appointment or remote doctor’s appointment: EUR 20.90. If the client has a consultation or consultations with other health care specialists on the same day in connection with a doctor’s appointment, they will only be charged the fee for the doctor’s appointment.
Nurse’s appointment or remote nurse’s appointment: free of charge
House call by a doctor: EUR 19.20
House call by another professional (such as a nurse): EUR 12.20
Rehabilitative day activities for seniors
- Part-day activities without transport (up to four hours): EUR 8.50
- Part-day activities with transport (up to four hours): EUR 14.10
- Full-day activities without transport (at least four hours): EUR 13.00
- Full-day activities with transport (at least four hours): EUR 18.60
The fees for part-day and full-day activities include a meal.
- Informal caregiver’s statutory leave as day activities: EUR 11.60. Includes a meal and transport.
- Remote day activities: EUR 5.00
Day activities for persons with disabilities
Meals are subject to a separate fee in day activities for persons with disabilities. Transport to day activities organised in accordance with the Act on Special Care for People with Intellectual Disabilities is free of charge. Transport to day activities organised in accordance with the Act on Disability Services and Assistance is subject to a transport service fee in accordance with the co-payments detailed in the Act on Disability Services and Assistance.
- Day activities organised in accordance with the Act on Disability Services and Assistance: meal and transport fee
- Day activities organised in accordance with the Act on Special Care for People with Intellectual Disabilities: meal fee, transport free of charge
Day activities for clients of mental health and substance abuse services
Meal fees:
- Lunch: EUR 6.00
- Snack/breakfast: EUR 2.50
Short-term inpatient care
Short-term inpatient care: EUR 49.60 per day of care. EUR 22.80 per day of care after reaching the annual upper limit (not charged to persons under the age of 18).
The fee for short-term inpatient care is charged for the day of arrival, days spent in the hospital and the day of discharge. If the client is immediately transferred to another institution, the sending institution is not allowed to charge a fee for the day on which the client is transferred.
Short-term assessment and rehabilitation period for seniors: EUR 49.60 per day of care. EUR 22.80 per day of care after reaching the annual upper limit.
Daytime or nighttime hospital care: EUR 22.80 per day.
This fee is charged if the client is in hospital care only during the day or at night.
Clients under the age of 18 can only be charged the fee for short-term inpatient care or daytime and nighttime care for up to seven days of care per calendar year.
Examination and treatment by a doctor at a detoxification centre: consultation fee EUR 20.90.
Inpatient detoxification: EUR 49.60 per day of care. EUR 22.80 per day of care after reaching the annual upper limit.
Rehabilitative inpatient care provided by substance abuse services: EUR 49.60 per day of care.
EUR 22.80 per day of care after reaching the annual upper limit.
Long-term inpatient care
An income-based client fee may be charged for long-term inpatient care if it is estimated that the service will be used for at least three months or if the service has been used for at least three months.
Long-term inpatient care is subject to a client fee based on the client's income. The client fee is 85% of the client's net monthly income, after statutory deductions. The client retains 15% of their net income or at least EUR 112 per month for personal expenses.
If the client lived in a shared household prior to the start of the service and their monthly income was higher than that of their spouse or partner, the client fee is determined based on the combined monthly income of the client and their spouse or partner. In this case, the fee is 42.5% of the total monthly net income, after statutory deductions. In any event, the client will retain at least EUR 112 per month for personal expenses.
Serial treatment: EUR 11.60 per session
The fee can be charged for up to 45 treatment sessions per calendar year. The fee is not charged to persons under the age of 18.
- Dental hygienist’s appointment EUR 10.30
- Dentist’s appointment EUR 13.30
- Specialist dentist’s appointment EUR 19.50
- House call by a dentist EUR 19.20
- House call by other health care professional (such as a dental hygienist) EUR 12.20
Procedure fees
Procedure fees are charged in addition to the basic fees listed above.
- Dental imaging EUR 8.50
- Panoramic x-ray of jaws and teeth EUR 19.20
- Preventive treatment: EUR 8.50 per visit
Persons over the age of 18 will be charged a fee of EUR 51.50 for uncancelled missed appointments.
Oral health care client fees are not charged to persons under the age of 18, with certain exceptions (orthodontic appliances that are lost or damaged due to incorrect use or negligence).
Oral health care client fees are also not charged to persons who have a service badge awarded to front-line veterans and war veterans, including to women veterans, or a certificate of participation in certain mine clearance operations. Oral health care client fees apply to everyone with the exception of the above-mentioned groups
Oral health care appointment and procedure fees count towards the annual upper limit for health care client fees (with the exception of costs associated with dental appliances, such as prostheses, dental guards, orthodontic appliances and sleep apnea mouth guards).
Oral health care client fees are based on a national decree on client fees. The invoice is sent to the client’s home after the appointment.
Example price list
The appointment fee is always charged. The appointment fee is:
- EUR 10.30, 13.30 or 19.50 depending on the recipient
Procedure fees are charged in addition to the appointment fee.
Examples of procedure fees:
- Oral examination EUR 8.50, 19.20 or 38.00 depending on the scope of the examination
- X-ray examination EUR 8.50 per x-ray, OPTG (panoramic x-ray of the jaws and teeth) EUR 19.20
- Local anaesthesia EUR 8.50
- Tartar removal EUR 8.50–55.60 per session
- Dental filling EUR 19.20–55.60 depending on filling size
- Root canal EUR 19.20–55.60 per session depending on the length of the session and tooth
- Standard tooth removal EUR 19.20–38.00 depending on the difficulty
- Surgical removal EUR 55.60–78.00
- Removable dentures starting from EUR 186.00 (each) + laboratory costs
- Removable metal-framed partial dentures EUR 225.70 (each) + laboratory costs
- Procedure fees vary from EUR 8.50 to EUR 225.70, depending on the degree of difficulty
Doctor’s appointment at a health station: EUR 20.90
Remote doctor’s appointment: EUR 20.90
Remote appointments are also subject to a fee. The fee is charged for the first three doctor’s appointments during the calendar year, including both remote and face-to-face appointments.
The appointment fee is not charged to the following groups:
- those under the age of 18
- persons who have a service badge awarded to front-line veterans and war veterans, including to women veterans, or a certificate of participation in certain mine clearance tasks.
When you arrive at the health station, be sure to present your service badge, annuity decision or Military Archives certificate at the patient office.
The appointment fee is not charged for doctor’s appointments that only involve providing health guidance or carrying out a health check-up. Such services include:
- maternity and child health clinic, school health care and student health care appointments
- preliminary call-up health examinations
- family planning appointments (if they do not include medical treatment)
- appointments in accordance with the Communicable Diseases Act (such as treatment of salmonella or sexually transmitted diseases), or
- mental health services, including various forms of treatment, provided as outpatient care in primary health care under section 27 of the Health Care Act are free of charge for clients of all ages.
Nurse’s and public health nurse’s appointments are free of charge. If a doctor's assessment is required in connection with a nurse’s appointment, a fee will be charged in accordance with the above-mentioned principles.
Doctor’s appointment fees count towards the annual upper limit for social welfare and health care client fees.
Meal fees
Meal fees are charged based on usage. Meal fees are charged when meals are not covered by any other fees charged to the client.
- Breakfast EUR 1.60
- Snack EUR 1.60
- Light supper EUR 1.60
- Lunch EUR 4.70
- Dinner EUR 4.70
Maintenance fees
Maintenance, centralised housing-related procurements by the unit and use of common facilities in housing provided by a disability services unit:
- The client’s maintenance includes 1–2 of the centralised procurements or uses of common facilities included on the list below: EUR 31
- The client’s maintenance includes at least three of the centralised procurements or uses of common facilities included on the list below: EUR 66.50
The centralised procurements and use of common facilities used as the basis for the client fee are as follows:
- internet connection or computer provided to clients by the housing unit
- daily newspaper and magazines in common facilities
- cleaning supplies, cleaning equipment and cleaning agents or toilet paper
- sauna use, if not included in the rent or subject to a separate sauna fee
- use of a washing machine, mangle or dryer in common rooms or a drying room
- use of shared recreational equipment or rec rooms, if not included in the rent.
The use of the aforementioned common facilities, supplies and equipment is defined in the plan prepared in collaboration with the client. The maintenance fee is charged if the client is present for at least five days during a month. The maintenance fee will not be charged if the client is present for less than five days.
