Home rehabilitation supports and guides you when it has become increasingly difficult to move around or accomplish everyday tasks. 

You can get support and guidance from home rehabilitation if it has become increasingly difficult for you to move around or accomplish everyday tasks due to illness, surgery or other sudden decline in the ability to function, and you are unable to move outside your home. Home rehabilitation is a form of rehabilitation that takes place at home and is linked to everyday life. The aim is to promote living as independently as possible at home and in its vicinity in order to make it possible for you to lead a meaningful everyday life. Examples of your own goals may be: - supporting/improving the ability to move (e.g. getting up from a chair, taking a flight of stairs or picking up mail from the mailbox) - daily chores (e.g. washing yourself, dressing yourself or cooking) - supporting medical care Health care professionals work in home rehabilitation and visits can be carried out either in the form of a treatment period or as a one-time visit based on a professional's assessment. The home rehabilitation period can also be carried out remotely with a specified device, for example, via a tablet computer or telephone. Home rehabilitation may include rehabilitation as well as medical and basic care. If necessary, the need for assistive equipment and small home alternations (support handles, thresholds/ramps), whose service availability varies by region, will be assessed. Together with the professional, a rehabilitation plan will be drawn up, which sets goals that support and motivate you in coping in everyday life. You need a professional's assessment to get a client relationship in home rehabilitation. The assessment can be carried out in the following situations, for example: - before being discharged from the hospital - through a home care or other health care professional - to support informal care by means of assessment, guidance or assistive equipment - based on a notification of concern. Home rehabilitation is carried out in close cooperation with your relatives and loved ones. During the period, the need for rehabilitation and/or further treatment is assessed.

Conditions for receiving the Service

Home rehabilitation is mainly intended for elderly people who have difficulties in performing everyday tasks and who cannot visit a health care facility outside their home due to poor mobility. The aim of home rehabilitation is to maintain and promote the meaningful ability to function in everyday life and to support safe living at home in a multiprofessional manner. You are eligible for home rehabilitation in the following cases: - Upon being discharged from the hospital, if you have reduced functional ability and need rehabilitation and/or medical help to support and assess your resources. - You are concerned about your or your loved one's mobility (e.g. difficulty getting up from a chair, taking the stairs and/or falling on multiple occasions) - You find everyday tasks challenging (e.g. washing, dressing and/or eating and drinking) - You or your relative taking care of you need assurance and guidance in maintaining your functional ability. (e.g. moving from the wheelchair to bed) The service can also be provided in the form of remote rehabilitation. Remote rehabilitation is suitable for you if you have been determined to need intensive rehabilitation and you are unable to visit a health care facility outside your home. Visits can either be combined with home visits or carried out completely remotely. The client relationship with home rehabilitation requires you to be motivated and committed to jointly set rehabilitation goals, taking into account the previous state of your functional ability. The progress of your rehabilitation will be evaluated together with you and your relatives/loved ones.
