Home rehabilitation for the elderly
Home rehabilitation supports and guides you when it has become increasingly difficult to move around or accomplish everyday tasks.
Home rehabilitation is mainly intended for older people whose coping in everyday life has deteriorated due to illness, ageing or other factors such as impaired functional capacity.
The aim of home rehabilitation is to maintain and strengthen your functional capacity in everyday life and to support your safe living at home. Home rehabilitation is carried out by a multiprofessional team.
Home rehabilitation may include rehabilitation, medical care and primary care. We will separately assess your need for assistive devices and minor home alteration work, such as support rails or threshold ramps.
A rehabilitation plan and goals will be drawn up for you together with a professional to support and motivate your coping in everyday life.
The progress of your rehabilitation will be assessed together with you and your family/loved ones. Home rehabilitation is provided for a fixed term and its duration and content are decided individually.
The service is subject to a fee. If you feel that you or your loved one needs home rehabilitation, contact Senior Info.
