Do you need many social and health services? In the future, your service package will be clearer

In life, you sometimes encounter situations where you may need more support and services than usual. Many different services can then become a difficult tangle that makes you tired and frustrated. What will happen next on my care path? Who is responsible for it, and who can I be in contact with? These challenges will be alleviated in Western Uusimaa. We are now looking for experts by experience to help us improve our services.

We are building clear care paths so that you can get help quickly even when you need many different services. There are challenges and problem areas in our current services that we want to address.  

“We want to help you in the right way and in time. When we identify your situation correctly, it helps us to take control and guide you to the right place,” says Project Manager Satu Meriläinen-Porras. 

Joint planning for a seamless service package 

It is important that everyone is able to influence the planning of their own care and things are not just decided somewhere. As a client, you also need clear information about how your treatment or service is progressing and what happens next.  

“When the service path is clear and the right people take care of your affairs, your treatment will not be prolonged unnecessarily and the problems will not get worse. You do not need to find out the way in the jungle of services on your own, because we are there to support and guide you,” Sari Lintunen sums up.

For our clients who need many services, we are now creating a model that adapts to the client’s needs. Have you been in a situation where you needed several different services at the same time? We are developing services in the Western Uusimaa social and health services programme. Municipalities are in charge of arranging services until the end of year 2022.
