Temporary home care for the elderly

We support living at home when our client’s ability to function is temporarily impaired and their need for care is short term.

Temporary home care is home care and home health care. Temporary home care is a one-time or short-term service that evaluates and strengthens the client's own resources. You can get into temporary home care based on a service need assessment. Temporary home care is granted to persons whose ability to function has weakened or the need for care is temporary/short-term. The implementation of care requires social and health care professionals, and the client is unable to seek outpatient health care, even with assistance, by using public transport or taxi services. Clients are guided to primarily use outpatient health care services and private service providers. The aim of temporary home care is to evaluate and rehabilitate the client. We set personal goals together with each client. The aim is to support the client with living at home independently as well as to encourage using their own resources. The service may consist of individual treatments, such as wound care, short-term support after procedures, safe implementation of medication as well as an assessment of coping in everyday activities. When possible, well-being technology is utilized to support treatment. Visits can also be carried out remotely. The service can be supplemented with support services, such as meal and safety aid services. Temporary home care is subject to a fee. Temporary home care can be provided directly by the wellbeing services county, as an outsourced service or with a service voucher with contract service providers. If the need for assistance only concerns housekeeping, assistance to run errands, we will refer you to use private services and help you organize them. Temporary home care services include home service as per the Social Welfare Act and home health care as per the Health Care Act.

Conditions for receiving the Service

Home care is based on the Social Welfare Act (Sections 19a and 20) and on the Health Care Act (Section 25). With temporary home care, we aim to safeguard your possibilities of living at home and to support you in restoring your functional ability. The service is intended for you if you have reduced functional ability due to illness, injury or other similar reason. Your functional ability is temporarily reduced, and you are still rehabilitating after surgery, for example. Your treatment requires the expertise of health care and social welfare professionals, and you are unable to seek outpatient health care services. You may also need services because your informal carer is temporarily unable to take care of you. The decline in the ability to function is typically noticeable in the performance of everyday activities, such as eating, washing, dressing, getting up from bed or a chair, mobility, personal hygiene, taking medication and daily activities related to housekeeping. During the treatment period, home care will assess your need for service, taking into consideration your previous functional ability. The service need assessment is conducted together with you and/or your relative(s). The service is based on supporting and strengthening your functional ability and personal independence. The functional ability assessment uses the RAI functional ability scale to assess the performance of everyday activities, daily decision-making and service needs. In addition to this, other assessment tools are used if necessary. A single functional ability assessment tool result is not a criterion for receiving assistance, support or service. Your need for service will be evaluated multiprofessionally with you, considering your overall situation. Your need for the service is non-recurring, occasional or temporary with a duration of a maximum of two months.
