Hospital discharge support from Hemma helps ensure a safe and smooth transition from hospital back home. The service is always planned individually according to the client’s needs and wishes, and family members can also be involved if necessary. The support can be provided for a fixed period or for as long as additional help is needed.
Visits are always unhurried and tailored to the client’s situation.

• Support in acquiring and using assistive devices
• Wound care, monitoring, and medication support
• Minor procedures such as suture removal
• Cooking, cleaning, and other household tasks

The service can be purchased privately or with a service voucher. The private share is eligible for the household tax credit. No separate charges for mileage or other additional or invoicing fees. Billing is only based on the time spent with the customer.
