If you or your loved one has experienced a safety incident while receiving care or service, we wish that you report it to us.
A safety incident can refer to harm caused to you or a near-miss incident involving, for example, communication or medication issues or problems related to care practices. We process all reports as part of our improvement efforts to prevent similar incidents in the future. The report is submitted anonymously. No response will be given.
