Mobile outpatient care services for adult mental health and substance abuse patients

The outpatient care services of the mental health and substance abuse services can be implemented partly as a mobile outpatient care service, in which case the service is brought to the customer's/patient's home and everyday environment. The service is goal-oriented, temporary and based on an individual client plan or treatment plan. The services are intended for mental health and substance abuse patients who have functional limitations due to their problems. The mobile outpatient care service is agreed and planned in cooperation with the customer and their network. The mobile outpatient care service is part of overall care and rehabilitation. It aims to help and guide the customer to cope as independently as possible in their everyday life. By working together to improve the everyday ability to function, we prevent social exclusion and reduce the need for hospital and institutional care.

Conditions for receiving the Service

The services are intended for mental health and substance abuse patients who have functional limitations due to an illness. The mobile outpatient care service is agreed and planned in cooperation with the client and their network.

The service is free of charge.
