Child welfare aftercare for young adults

We provide support for a child or young person - and their network - after foster care or other protective placement ends.

We support young adults who have been taken into care or placed in substitute care for more than six months. We help them with matters related to independent living and the transition to adulthood after substitute care.

We support young people with housing arrangements and independent living, such as finding a place to live and managing everyday life. We also support them in health matters and in maintaining social relationships. We provide guidance on financial matters and benefit applications, and help with studies, job search and life planning. We also support young people through different challenges in life. If necessary, we can also offer parenting support to the young person's parents.

The service is voluntary for the young person. If a young person's after-care has ended, the service can be restarted if they wish. After-care ends at the latest when the young person turns 23. After this, we will refer the young person to other services if necessary.

After-care clients from other wellbeing services counties who are moving to Western Uusimaa

Young people receiving after-care who move to Western Uusimaa from another wellbeing services county receive the same after-care as our other clients. Their after-care social worker may contact the leading social workers for further instructions on submitting a transfer request.

After-care for young people living in other wellbeing services counties whose costs are the responsibility of Western Uusimaa Wellbeing Services County

We have appointed an after-care social worker for each young person whose after-care costs are the responsibility of Western Uusimaa Wellbeing Services County. Our social worker is responsible for cooperation with the wellbeing services county of the young person’s home municipality. For billing details, see: Invoicing instructions
