Treatment of chronic diseases

In the case of chronic diseases that require monitoring (e.g. asthma, diabetes, coronary artery disease, hypertension), the goal is to achieve good therapeutic control. As a patient, you have an important role in keeping your chronic disease under control.

Contact your health station to:

  • Begin treatment and create a care plan
  • Schedule follow-up appointments according to your care plan
  • Address changes in your condition identified through self-monitoring

Routine Check-Ups

To schedule a check-up, contact your health station. A nurse—or a physician, if needed—will assess your condition based on your individual needs. Appointments can take place remotely or in person. 

When booking your appointment, you will also receive instructions on how to prepare for the appointment.

Diabetes: How to prepare for your appointment

Patients with type 2 (adult-onset) diabetes receive treatment at health stations. Most individuals with type 2 diabetes do not need to monitor their blood sugar at home. A diabetes nurse will evaluate whether home monitoring is necessary for you, and, if so, provide a referral for free supplies.
