Regular home care for the elderly

We help the elderly live independently at home.

Regular home care supports living independently at home and encourages the client to use their own resources. Home care is organized based on a service need assessment and as per the client's care and service plan. The plan is assessed regularly, where services can be added, removed or discontinued based on the client's needs. A nurse-in-charge is appointed for the home care customer. The home care service includes home care as per the Social Welfare Act and home health care as per the Health Care Act. Service is available for our clients round the clock. Home care can be carried out as home visits or as remote visits. As part of the home care services, well-being technology, such as electronic door opening and medication dispensers, is utilized. The home care service includes supporting the client's ability to function and independence, taking care of nutrition and personal hygiene as well as medication and medical procedures and follow-up. Regular home care is granted based on a service need assessment and if the need for services is daily or recurring at least once a week. Home care can be provided directly by the wellbeing services county, as an outsourced service or with a service voucher with contract service providers. We draw up an individual treatment and service plan in cooperation with the client, which we implement with a rehabilitative approach together with your close family members/persons. We record in the plan individual goals and means for actions that the client cannot perform independently. We guide and encourage clients to use their own resources and support them in maintaining their ability to function. Regular home care is subject to a fee. The monthly fee is determined based on the client's treatment and service plan. The fee is also influenced by the size of the household and the total gross income. Based on the service need assessment it is also possible to get support services to support living at home.

Conditions for receiving the Service

Home care is based on the Social Welfare Act (Sections 19a and 20) and on the Health Care Act (Section 25). Regular home care is intended for you if you have a reduced functional ability due to ageing, illness or injury, and you are unable to cope independently, with the help of relatives or other services in everyday life. The aim with the service is to ensure your possibilities to live at home as long as possible and to support you in maintaining your functional ability. Your treatment and care require regular and long-term expertise from health care and social welfare staff. Home care is provided to persons of all ages, whereas home care is provided to adults. The decline in the ability to function is typically noticeable in the performance of everyday activities, such as eating, washing, dressing, getting up from bed or a chair, mobility, personal hygiene, taking medication and daily activities related to keeping the home environment clean. The service is granted based on an assessment made by a professional. The service needs assessment is conducted together with you and/or your relative(s). When assessing your functional ability, RAI (Resident Assessment instrument) is used as an assessment tool. With this tool, we assess the performance of everyday activities, daily decision-making and service need. If necessary, other assessment tools measuring health and functional ability will be used. The result of a single assessment tool is not a criterion for receiving assistance, support or service. Your service need will be evaluated considering your overall situation. With the help of the criteria for regular home care, we allocate services to those who need the most support for coping at home due to reduced functional ability. The need for service must be continuous (weekly or daily) and last for more than two months.
