When can you apply for disability services?

Disability services are meant for you if a disability or long-term disease makes your everyday life difficult, and other services are not sufficient to support your functional capacity. For example, the services may be related to mobility, housing, assistance, communication or work.

You can apply for disability services when a disability or long-term illness that has lasted for more than a year complicates your daily activities, participation in society, studies, work or mobility.

Applying – electronically or on paper

There are two ways to fill in an application for disability services. The smoothest way is to fill in an electronic form, for which you need your online banking codes or mobile certificate. If you cannot use the electronic form, you can print the form out from the website and return it to Lunna(external link) to the disability services mailbox or send it traditionally as paper mail. You will find both the electronic and the printable application form on the disability services website.

What accompanying documents do you need?

In your disability services application, you will need to attach a document that shows the information related to your disability or disease and how it affects your daily life. This is important so that we can assess your service needs individually and so that the service decision will be based on up-to-date information.

Information on the disability or disease must be attached to the application. You can do that, for example, by printing out or taking a screenshot of an entry stating your disability or disease in the Kanta services.

Find an entry related to your disability or disease, such as a medical statement, medical record and/or record of visit.
Print the specialist statement or document concerned and take a clear screenshot of it.
You can either attach the file to your application in the electronic system or enclose the print with your application on paper.
A new medical statement is usually not needed as the effects of your disability or disease on your functional capacity may be indicated in the latest medical statement or medical record entered by a healthcare professional.

You can also provide your own description of how the disability or disease affects your daily life as an attachment to your application. This will help the people granting the services to understand the challenges and individual needs you have in your daily life.

Write an informal description of how the disability or disease complicates your daily activities, mobility, communication, studies, work or social life.
Give concrete examples of everyday situations in which you need help or support.
in matters concerning home adaptations/support for accessible housing, you need to first contact an occupational therapist who will assess the necessity of the home adaptation.
You can write your own description in the application or as an attachment to it. If you need assistive devices, the contact details of the assistive equipment units of different municipalities are available here.

Check all information and documents before sending the application. We can process the application faster when the information in it is correct and up to date,

Provide your up-to-date contact details in the application so that we can contact you or ask for additional or supplementing information.

It is also possible to supplement the application afterwards if you notice that some important information is missing.
