Granting social and health care services
On this page, you will find information about assessing service needs as well as the principles, conditions and criteria for granting social and health care services.
The aim of services for older people is to support living at home and maintaining functional capacity for as long as possible. The granting of services is always based on an individual service needs assessment and a client plan. Services may be granted for a fixed term or continuously, and they may be increased, reduced or discontinued if the client’s functional capacity and/or service needs change.
Who can be granted services?
Services may be granted to persons whose functional capacity has been reduced, for example due to advanced age, illness, disability or another reason, or who need support due to a particular family or life situation. A prerequisite for granting services is that the client actually lives in a municipality within the Western Uusimaa Wellbeing Services County, in accordance with the Municipality of Residence Act. In urgent situations, necessary care will also be secured for persons residing in the wellbeing services county.
How to apply for services
As a new client, you can apply for services by contacting Senior Info. If the client is already a client of services for the elderly, they should contact their appointed worker or the service provider.
The service needs assessment is carried out comprehensively and without delay. For persons over 75 years of age, the assessment must be initiated no later than on the seventh working day from the contact. The assessment is free of charge to the client. The overall assessment considers, among other things, the person’s ability to handle day-to-day life, physical functional capacity, psychosocial well-being, obstacles related to the living environment, financial opportunities, and the opportunities of loved ones and volunteers to support the person’s day-to-day life.
The service needs assessment utilises the RAI assessment tools as well as other reliable indicators that measure functional capacity and care needs. A single indicator result does not alone determine whether a service is granted; the decision is made based on the client’s overall situation, in cooperation with the client and, if necessary, their family and other loved ones.
