Granting social and health care services
On this page, you will find information about assessing service needs as well as the principles, conditions and criteria for granting social and health care services.
The aim of services for older people is to support living at home and functional capacity for as long as possible. The granting of services is always based on an individual assessment of the need for services and a client plan. Services may be granted as fixed-term or continuous services and they may be increased, reduced or discontinued when the situation changes.
Who can services be granted to?
Services may be granted to persons whose functional capacity has declined because of, for example, advanced age, illness, disability or some other reason, or who need support because of a specific life situation. The precondition for granting services is that the client lives within the Western Uusimaa wellbeing services county, in accordance with the Municipality of Residence Act. In urgent situations, necessary care is also ensured for a person residing in the area.
How to search for services
A new client can apply for the services by contacting Senior Info. If the customer is already a client of the services, they can contact their own contact person or the party delivering the service.
An extensive assessment of the need for services will be carried out without delay. If the client is over 75 years of age, the assessment will start no later than within 7 working days of the contact. The assessment is free of charge for the client. In the assessment, matters such as coping with everyday life, physical and psychosocial functional capacity, the living environment, the financial situation and the opportunities of loved ones and the client’s network to provide support are taken into account.
The RAI assessment and other indicators are used in the assessment. An individual indicator result alone does not determine whether the service will be granted. The decision will be based on the overall situation.
