Acting on behalf of another individual in social and health services

If managing your personal affairs is challenging, you can authorize someone else to handle your matters with social and health services on your behalf. This page is being updated.

Acting on behalf of another individual (Health stations)

You can authorise another person to act on your behalf by providing them with a power of attorney.

The power of attorney must include the following information:

  • The name and personal identity code of the authoriser.
  • The name and personal identity code of the authorised person.
  • The duration of the authorisation, specifying the start date. The authorisation can also be valid until further notice.
  • The specific actions the authorised person is permitted to take on your behalf.
  • The authoriser’s signature, location and date.

Please hand in the signed power of attorney in person at the health station. The authorisation and any limitations will be recorded in the client information system. You may authorise someone for a fixed period of time or until further notice. You can revoke the authorisation by notifying your health station in writing.

Please note that authorisations granted in the Suomi.fi service to act on behalf of another person do not give permission to act on behalf of another person at a health station. The authorisation can only be used in our digital services.

To act on behalf of another person at a health station, you always need a separate authorisation, personally handed in at the health station.

Digital services (for instance appointment booking online)

You can authorise another person to act on your behalf in digital health services by giving your consent with a Suomi.fi authorisation.

Note: This authorisation does not cover services at regular health stations or phone services at health stations – For this purpose, you need to use a separate authorisation.

Acting on behalf of another individual (Disability Services)

If you find it difficult to manage your personal affairs, you can authorise somebody else to handle your matters with Disability Services on your behalf. An authorisation is always voluntary.

You can give the authorisation by filling in a form which you can find at the bottom of this page. You can draw up a written free-form power of attorney provided that it includes all necessary details about the authoriser and the authorised representative, and information specifying the matter for which the power of attorney is prepared.

You can authorise another person to handle the following matters with the Western Uusimaa Wellbeing Services County Disability Services on your behalf:

  • submit applications and appendices related to disability services
  • obtain information about the processing of an application
  • obtain information about the content of decisions regarding disability services
  • obtain information about other matters specified in the authorisation
  • attend meetings on your behalf

Please sign the form and deliver it to the Disability Services where the authorisation and its limitations will be entered into the client data system.

You can cancel the authorisation by filling in a cancellation form and delivering it to Disability Services. You can find the cancellation form at the bottom of this page.

Please note that a service provider cannot act on behalf of the client using a power of attorney in matters that affect the service that is being provided.
