Service needs assessment – the client’s path to services
Do this if you are using the disability services for the first time.
First contact the customer service
If disability services are needed by you or someone close, please contact the Social Services Guidance. You can contact the customer service by telephone or digitally in Lunna. The contact information is provided on this page.
If you are over 65 and have never been a client of the Disability Services, please contact Senior Info first.
Then make an application
A customer service worker will tell you how to file an application for disability services. Fill in the application and deliver it to the Disability Services with the required attachments.
A disability social worker will begin to process your application without delay, no later than within seven days. The worker will then contact so that you can set up a meeting for a service needs assessment.
You can express your wishes at the meeting
The meeting is usually held in the client’s home. You can also invite family members and friends to attend. During the meeting, you can tell the worker about your functional capacity and service needs. They worker will write down your wishes and tell you about the services.
The service needs assessment or client plan is completed
After the meeting, the worker will assess the services you need and send you a service needs assessment or client plan by e-mail. If you would like to make changes to it, you can enter them in the plan. Send the plan back to the worker within three weeks.
The service needs assessment is now complete, and you will receive a decision on the services without delay, but no later than within three months of submitting the application. Your services will then commence according to the service needs assessment and client plan.
