Need for a rehabilitation plan
You can apply for intensive medical rehabilitation through Kela. In some cases, your insurance company may require a rehabilitation plan from public healthcare services.
The aim of intensive medical rehabilitation is to help you to be able to work and/or to improve your daily functioning despite illness or disability.
You may be eligible for intensive medical rehabilitation if:
- You are under 65 years old.
- An illness or disability significantly affects your daily life and ability to participate in activities.
- Your rehabilitation needs will last at least one year.
- The rehabilitation is not part of direct medical treatment.
- The goal of the rehabilitation is to enhance your functioning and participation, not to provide medical treatment.
- You are not in public institutional care.
For intensive medical rehabilitation, you will need a rehabilitation plan. This plan is prepared by your doctor in public healthcare. You should discuss with your doctor your wishes and goals for rehabilitation and how these can be incorporated into your daily life.

Your insurance company may require a rehabilitation plan from public healthcare services (for example, after a workplace or traffic accident). If this is the case and you are eligible for rehabilitation covered by your insurance, please first contact the rehabilitation planner in your wellbeing services county.
Contacting A Rehabilitation Planner
Rehabilitation planning is a low-threshold service and does not require a referral. Rehabilitation planners can guide and advise you on rehabilitation-related matters and coordinate your rehabilitation plan and any required assessments.
Rehabilitation planners assist individuals aged 18 and above whose rehabilitation is handled by primary healthcare. In addition, they assist people of all ages with intellectual disabilities, provided the diagnosis was made before the age of 18.

Booking Appointments For Therapy Assessments
The rehabilitation planner will coordinate your rehabilitation plan and provide you with advice and guidance on booking your therapy assessments. Upon confirming your appointment, you will receive a message with detailed instructions, including service location and addresses.
- Request a feedback statement from your rehabilitation therapist 2 to 3 months before your rehabilitation period ends.
- Please also request any relevant statements from schools, work and day activity centres, or housing units.
- Bring the statement to your therapy assessment and send it in advance to the rehabilitation planner.
- You may also request your rehabilitation therapist, teacher, or instructor to send the statements directly to the rehabilitation planner.
- Provide the most recent medical records or statements from specialized healthcare that may influence your rehabilitation to the rehabilitation planner or doctor.
- If you move and your wellbeing services county changes, please ensure you have all necessary documents and plans from your previous wellbeing services county.
At your doctor's appointment, you will need to confirm the validity period of any benefits you receive.

Therapy assessments and doctor’s appointment
As part of your rehabilitation plan, you need to attend therapy assessments within primary healthcare services. These assessments will evaluate your current functional capacity. Based on these assessments, you and your therapist will plan the goals, content, duration, and frequency of your upcoming rehabilitation.
Remember to bring any necessary assistive devices, such as glasses, hearing aids, or mobility aids.
Furthermore, please ensure that the rehabilitation planner has received a statement from your rehabilitation therapist. If unsure, bring the statement to your assessment.
The doctor's appointment will take place after your therapy assessment. This ensures that both you and your doctor have access to the assessment results when finalizing your rehabilitation plan.
Submitting your application to Kela
To apply for rehabilitation, fill out the KU104 form. Note that this form must be filled and submitted once a year, even if your rehabilitation plan is valid for 2–3 years.
