Social work in housing services for mental health and substance abuse clients

Through this service, we refer mental health and substance abuse patients to housing services. We assess what kind of housing-related help clients need and help them access the right services.

Social work is systematic customer and expert work carried out as per the Social Welfare Act. Social work is responsible for assessing mental health and substance abuse clients' service needs and referring them to housing services. The housing service you receive via social work is integrated with the support provided by other operators and the implementation and effectiveness of the service is guided and monitored. In the service, your service needs will be assessed together with you, and you will receive a decision on housing services as per the Social Welfare Act. The service needs of customers referred to housing services are evaluated on a regular basis. A service plan is drawn up with customers, which describes the objectives of rehabilitation and the means to achieve them. Social work is free of charge. The aim of the service is for you to receive the housing services and rehabilitation that you need, in order to be able to live as independently as possible. Seeking these services is voluntary for you. If you need support for housing and have challenges related to mental health and/or substance abuse problems, you can contact the mental health and substance abuse services. Your relatives or carers can also contact these services. The service is temporary, and its duration depends on your service needs.

Conditions for receiving the Service

The service is intended for adults who need housing support due to reasons related to mental health or substance abuse. The client applies for the service by contacting Mental Health and Substance Abuse Services, or the contact pursuant to the Social Welfare Act can be initiated by an authority or a person close to the client who is worried about the client's housing situation. A referral is not required. The service needs assessment of clients is performed in accordance with the Social Welfare Act.

The service is free of charge.
